Mid Suffolk Key Saver Terms and
Conditions |
- Mid Suffolk Key Card means Mid Suffolk Key Card Project, c/o
Mid Suffolk District Council, 131, High Street, Needham Market,
Ipswich, Suffolk IP6 6DL
- Mid Suffolk Key Project is a partnership between Mid Suffolk
District Council and Suffolk County Council
- Any individual can apply for a Mid Suffolk Key Card provided
that they are eligible.

- Only the named Cardholder may take advantage of the Mid Suffolk
Key Card. It is not transferable
- The Card will need to be shown whenever the Cardholder wishes
to take advantage of the services offered by Mid Suffolk Key
Card
- The Card can only be used according to the terms and conditions
laid down by the individual services offered on the card
- Cards that are lost, stolen or damaged can be replaced
- When a Cardholder wishes to change details other than their
name, amendments will be made after fulfilling authentication
requirements
- Mid Suffolk Key Card must be notified as soon as possible of
any change of entitlement to either the Mid Suffolk Key Card or the
services offered on the Cardholder’s Mid Suffolk Key Card
- It is the responsibility of the cardholder to provide Mid
Suffolk Key Card with correct information
- All lost or stolen cards must be reported to Mid Suffolk Key
Card as soon as possible
- Mid Suffolk Key Card reserve the right to withdraw or amend the
Mid Suffolk Key card at any time
- Mid Suffolk Key Card will not be liable for any mistakes or
omissions made to the cardholder’s data at any time
- Periodically Mid Suffolk Key Card may wish to contact you to
ensure that the information we hold on you is up to date and
request your help in the development of the scheme
- The Mid Suffolk Key Card remains the property of Mid Suffolk
Key Card Project and must be returned on request
- All data collected is in accordance with the Data Protection
Act 1998
- You may ask at any time to be sent a printout of the data being
held on your behalf

Freedom of Information Act relating to the Mid Suffolk Key Card
